Our Services

Disease Management Program Coming Soon!

Health Support: Registered Nurses

  • Phone call weekly from your assigned Registered Nurse to assess compliance with care regimen
  • Phone access to Registered Nurse as needed for general health questions. (These nurses will not provide a diagnosis or recommend medical treatment. They will only make recommendations).
  • Pre-Office Visit consultation by phone as needed
  • One in home medication safety visit monthly as needed
  • Healthy Pregnancy, Healthy Baby Program
  • General Women’s Health Consultations
  • COVID-19 support based on CDC recommendations

Health and Wellness – Certified Nutritionist, Life Coach, and Exercise Instructor

  • Certified nutritionist will be available by phone for consultation with dietary restrictions, plans, and goals
  • A Certified Life Coach will be available for phone consultation for assistance with coping with chronic diseases and devastating diagnoses.
  • Nutrition Classes that come to you if you have a group of 10 or more
  • Exercise Classes that come to you if you have a group of 10 or more

Educational Support – Certified Teachers

  • Provide telephone consultation for school-aged children in K through 5th grade after school and on Saturdays
  • Weekly phone check-in for accountability and support
  • One home visit allowance per month for Special Projects
  • Parental coaching via telephone for improvement in at-home support
  • Virtual Learning Support
